When you on an active phone call, do you not have the option for "Speaker" but instead "Audio”? If so, this happens when 1) you have a Bluetooth connected speaker system (including headphones, cars, speakers, etc) or 2) have a nearby iCloud connected (using the same iCloud account) Mac or other iOS device.

That is definitely a speaker I just listened to it on both AirPods (just now and in the past)I assuming you have AirPods and you can see where the speaker is located on the outer side of the AirPod. There is not debris in the internal speakers I've already been there and done that.
